Treatment of Severe Diabetes Mellitus with Continuous Low - Dose Infusion of Insulin

Abstract

|
Standard treatment of diabetic ketoacidosis and severe hyperglycemia consists of the administration of initial large doses of insulin with proportion given intramuscularly or subcutaneously and the rest intravenously followed by repeated injection at irregular interval. In recent years good results have been reported in the treatment of severe diabetes mellitus with the continuous intravenous infusion of small doses of insulin.
The present study is to report the results of continuous low-dose infusion of insulin in the treatment of 3 diabetic ketoacidotic and 4 hy¢¥perglycemic patients.
1) Intravenous infusion rate of insulin to achieve. normal range of plasma glucose was 1.7-6.9 IU per hour.
2) The total insulin dose needed to attain normal glucose level was 15-76 units given over 6-15 hour period.
3) The mean rate of fall of plasma glucose was 41.8 mg% per hour.
4) Plasma potassium was well preserved within normal range and profound hypoglycemic attack was not present throughout treatment in all cases.
